Doctor, I am 30 years old male, and have a complaint of having off and on pain in my upper abdomen usually starts after intake of food. I also have gas in my stomach and sense of indigestion. Please solve my problem.

You have dyspepsia (acidity). I advice you to reduce chili food intake, take more yogurt and milk. I hope the matter will solve. If it won’t, then start taking Syrup Mucaine 2 tablespoonfuls 3 times a day 15 minutes before meal + tablet Zantac 150mg 2 times a day for 10 days.


Am 42 y/o female. Started week & half ago with "stuffed headache" type feeling. Not painful, but nonstop 24 hours. Went to the ER, They checked B/P (always low) pulse and blood pressure. Everything was okay. Not knowing what else they put me on anti-inflammatory and nose drops for possible sinus infection. No previous cold, recent allergies or stuffiness. Have taken for 4 days now. My Only serious medical history is a TIA a year ago with no possible cause. Family history of stroke and heart problems from mom and TIA from dad. Should I be concerned and if so, please tell me what kind of tests I should have done. The "headache feeling" is centered in front half of head with eyes feeling tired as a result as well. Get your complete checkup. I advice you to have following tests: Blood Complete picture, Fasting Blood Glucose, Lipid Profile, ECG, X-ray PNS, Eye Checkup. Start taking 1/2 tablet of Aspirin daily in morning after meals. I am unable to figure out your problem with such a short history. You need to have your tests done first to find out what’s the cause behind your problem



I have had recurring diarrhea for nearly four weeks now, off and on. I'm certain it is partially due to stress, diet, and lack of sleep. I have since cut out alcohol, coffee, and milk; however, my system still seems to be 'sensitive'. I am not experiencing any pain in my stomach, so I figure the problem lies in my bowels or intestines. Any advice would be appreciated, as I have already dropped close to ten pounds as a result of this problem.

Please tell us what is your age? How many times you go to toilet? Is there any blood in your stools? I advice you to get your medical checkup and stool routine examination. Take lot of water and food containing lot of fiber like vegetables and fruits.



My Father is of 52 years of age. He is suffering from kidney/blood infection. His blood contains on an average 70mg/dl of Urea. And Serum Creatinine is also above 4.5 mg/dl. He is a heart patient also. Sometimes he eats something and that does not digested and he just vomits all out. Please suggest what his diet should be? What sort of precaution he should take? What he should to let urea and Serum Creatinine down? Can you tell me what Serum Creatinine is and what it does?

Your father is suffering from Renal Failure. Get him admitted into a hospital. He might need treatment for lower his urea and Creatinine down (but not dialysis at these readings). Give him protein and salt less diet. Creatinine is a waste product of protein metabolism that is found in the urine. It can be measured to assess overall kidney function. An abnormally elevated blood Creatinine level is seen in those individuals with kidney insufficiency and kidney failure.



My father is 58 yrs old. He is suffering from hypertension for past three years. He initially took regular medication for it and his B.P. was controlled. Now for past few months he has stopped taking medicines but his B.P. is controlled and only twice did it go up. Please tell me does he need to take regular medicines or is it fine without taking medicines.

Although your father's B.P. is under control but it can rise any time because your father is suffering from hypertension. He must take regular medicines as advised by the doctor. High blood pressure, if not treated, can give rise to various complications which can also be life-threatening. You must get your father's blood pressure checked up regularly and should also continue taking the medicines.



Hello. My age is 40 years, and few days back I started having heaviness in my chest while doing some work. It relieves after taking rest. What’s the problem?

You are having Angina on effort (pain/heaviness in chest after doing work). If you are smoker, stop it; avoid fatty diet (cholesterol) in your food. Take more vegetables instead of meat. Start Tab Disprin 1/2 Tab per day in the morning after breakfast. And also carry tablet Angicid in your pocket and whenever you feel heaviness/pain, put it under your tongue. Have your ECG done during Pain. Get a complete checkup.



What are the main changes that occur in the ECG after heart attack?

ECG Changes after Heart attack or Myocardial Infarction (MI) are: ST-T Changes: Convex St Segment elevation with either peaked upright or inverted T-waves is usually indicated of acute injury. ST segment depression, particularly when it persists may also be indicative of a non-Q wave MI. Q-waves: The development of Q-waves (> 40 m/sec) is generally considered diagnostic of MI but may occur in patients with myocarditis and prolonged ischemia. MI may also occur in the absence of Q-waves (Non Q-wave MI). ST Segment may come to normal after week with the permanent appearance of Q-waves.



Hi I’m 19 years old and I am quite thin and slim as compare to others of my age, is there any thing which helps me gain mass. I saw a medicine named century 21 adult weight gain, can I use those tabs and are they safe.

Improve your food intake, take frequent meals, do heavy exercise (body building in Gym), don’t take any tension and be happy. I advice not to use any medicine which is available for weight gain. If you have less appetite, it'll get improve once you start exercise because more you exercise more your body needs food and more the appetite. Food you use should be high caloric (meat, butter, milk, fruits).


Heavy exercise for 1/2-1 hour per day would be enough to build up your body. Get help from an expert (present usually in all gyms) and he'll guide you properly.


In follow-up to "chronic diarrhea" question posted earlier today: To answer your follow-up questions, I'm a 29-year old male. I go to the toilet about once a day unless I've eaten something that is irritating me, and then I tend to go frequently until I pass everything I ate. I am not noticing any blood in my stool. Additionally, I have not been traveling, so I know it's not a 'traveler’s illness'. Could my problem be viral, or is it more likely I have damaged my digestive system through abuse/malnourishment? Thanks.

As I advice earlier, take fiber diet. Is there any history of emotional stress or anxiety? Start tablet Lomotil twice daily and Tab. Metronidazole 400mg 3 times daily for 5 days. If this won’t help you after 5 days, get your stool routine examination, erect abdominal X-ray, and thorough checkup. You might need your rectal examination (proctoscopy, sigmoidoscopy) and biopsy.


Sir/s I want to know about hepatitis C. Is there any medicine available here in Pakistan other than injections? If available to whom should I contact for checkup and treatment in Islamabad/Rawalpindi. I am asking that one of my friend’s aunt was suffering from this disease after consultation with the doctor and his prescribed drugs (tablets) her medical test showed that the virus was cleared. Unfortunately, my said friend his out of the town. Thank you for your help and advice.

If your aunt's Hepatitis C virus gets cleared (negative viremia by PCR and normal ALT), there is no need for any further treatment. However, if she has a positive active Hepatitis C virus with no signs of Liver decompansation, she is a candidate of Injection Interferon along with Oral Ribavarin (a dual therapy). There is no single oral therapy for Hepatitis C. Get her PCR test, ALT done and send me her latest values. You can consult any renowned Gastroenterologists in Rawalpindi/Islamabad for more information.

I am 31 years old and facing a problem of anus swelling like in Piles with very small bleeding. Please advice some medicine for this. Thanks Baber

Dear Baber, Piles is a very common problem in our country and the cause of it is smoking, pollution, constipation etc. If u think your disease is at an early stage, the best thing to do is to find out the cause of your piles and once you have done that try to avoid it.
Piles can be very troublesome on occasions so I suggest that you see a surgeon at this stage and get maximum help, as the diagnosis and stage of the disease is best made on examination. So my advice to you is to see a doctor and get it checked.


AOA! Dear Doc, I have a problem. I have been taking tablets for peptic ulcer for last 1 year. But for the last three months I have blood in my stools. Since then I have lost weight and I also have lost my appetite. Can you please advice me in this regard?

First thing I need to know is that which Tablets are u taking for peptic ulcer. I would also like to know that are you taking it on regular basis. About the blood in stools you have to answer the following questions:

  • Is it fresh blood?
  • Amount of blood?
  • Does the blood come before/after or mixed with stools?
  • Are u taking any other medications along with Tablets for peptic Ulcer?
You have to take some tests so that diagnosis of your disease can be made out. Please have your Blood CP and stools for occult blood done.


I am a known diabetic for the last 10 years. I am on oral medication since then. I have a wound on my foot for last 1 month which is not healing. I have taken antibiotics for my wound but this wound seems to me to be increasing. What should I do?

Consult your doctor immediately. As u have mentioned that you are diabetic for the last 10 years it is most likely that you have a diabetic foot which means that you have an ulcer on your foot which is not healing due to uncontrolled blood sugar levels. You need to have your sugar levels done regularly, diet control, exercise, change or increase of your medication according to the doctor's prescription. However the appropriate treatment would be cleaning of the wound with daily dressings by the doctor.


Dear Doc, I am a 50 years old male. Recently I have noticed that I have to go to bathroom 9-10 times a day. Moreover I have feeling that I cannot empty my bladder fully. I also have a burning sensation. I have gone to a local doctor and he has advised me Tab. Urixin but my complaints are still there after taking the treatment for a week. Kindly can u help me in this regard?

Your local doctor has been treating you for the suspicion of urinary tract infection. Tab Urixin which contains pipedemic acid is a urinary antiseptic. It may have finished your complaints of burning sensation while micturating but your symptoms and age suggest that you have problem other than urinary tract infection. At ages of 50 and above, there are physiological (hormonal) changes in a gland (prostate) in males which leads to its enlargement which causes the symptoms of going to toilet frequently, and many times at night, not able to control the desire to micturate, weak stream of the urine, dribbling of urine after micturition, sensation of incomplete bladder emptying. In this regard you have to have a per rectal exam done by a doctor and then an Abdominal ultrasound for prostate.Kindly send me the details of your per rectal exam and report of ultrasound so I can advise you regarding your problem.


Dear Doc! I am 19 years old. I had a small swelling on my wrist which was removed 4 months ago. Now it has reappeared again. It looks too ugly and sometimes I also have pain in my hand. Kindly help.

As you have described it, it looks as if you are having a ganglion on your wrist, which reappears even after it has been excised (removed). You have to go to your doctor again for the removal of the swelling or have Injections of corticosteroid injected into your swelling.


I have a severe neck pain in left side the x-ray report shows: There is loss of normal cervical curvature. - Conclusion: - Loss of normal cervical curvature due to may be due to muscular spasm the pain comes down to left hand above elbow due to this I can not use my left hand which is in serve pain. I am taking following medicine since 5 days and physiotherapy but I am still in pain. I am taking: - Voltral SR 100, Feldene 20mg, Ternelin 2mg please advise your comments/suggestions

Take either Voltral SR 100 or Feldene, because both are same type of painkillers. Avoid taking these medicines before meals. Try Cream Capcidol and apply it locally 2-3 times / day. Do regular physiotherapy. Homeopathic also helps in this regard. But don’t take both therapies at the same time.


I woke up with pain in my underarm. (For 2 days so far.) So I checked and noticed that there is a small lump there. (As big as half the nail of a person's little finger.)When I press/stretch the part of the flesh where it hurts it kind of takes the look of a white-mosquito bite. It only hurts when I have my underarm near my body or if I touch it. I thought maybe it could be a boil, but boils are usually red, & it's not an ingrown hair. & now I'm worried wondering if it could be cancer. I didn't think lumps from cancer were supposed to hurt like that. But cancer so young? I guess it's possible.

What’s your age? Are you having Fever with it? According to your description, it seems that the swelling is a developing boil. Boils are common in underarm. It’s not necessary that boils are always red. You can use a simple painkiller like Aspirin for that.


I am 23. I am feeling some pain and a small swelling near my anus. Some time puss discharges and I also feel itching. There is no effect of any NSAID or antibiotic. Please tell me what is that and how is it curable. And I feel comfort after discharge of pus.

You are suffering from Perianal Abscess. The only effective treatment for this condition is the surgical drainage of Abscess. You need to consult a surgeon for that. Antibiotics and Pain killers are effective only after the removal of abscess.


I am having some foul-smelling blood-containing discharge from navel/umbilicus for the last 3-4 months. Earlier I was treated for some bacterial/fungal infection but the discharge still continues. One doctor has diagnosed it to be UMBILICAL GRANULOMA and advised CAUTERIZATION w/o any proper test! As a matter of fact I had laparoscopic appendicitis surgery 3 yrs back. Please advice. There is no need for any further test at this time. You should go for cauterization which is curative in most of the cases but if your condition recurs after the cauterization, then there will be need for exploration of wound to look for any underlying disease in the abdomen. But there is much possibility that this disease will recover after the cauterization.


For the last 5 years I have been experiencing bleeding with defecation. Initially it was small in amount. But for the last three months I have noticed an increase in amount of the blood and something coming out of anus?

I would like to know more about the symptoms you are experiencing, Let me know your age, the color of your blood Fresh (red) / black),any symptoms of constipation, pain during defecation. However with your description it looks that you have third degree hemorrhoids which will require surgery eventually. You must undergo battery of specific investigations which include per rectal examination, proctoscopy and sigmoidoscopy.

Hello Doc, I have 2 children age 1 and 3. Both had normal deliveries. Now my wife is 8 months pregnant. She is doing fine; movements of the baby are also good. However, for last 1 day she is complaining of slight bleeding off & on. Do advice on this condition.

Dear Client, This is a typical case of Ante partum Hemorrhage and as suggested by your history most probably this is due to Placenta Previa. It is a very dangerous condition and can lead to massive bleeding and can jeopardize health of both mother and baby. I strongly advise you that you URGENTLY see some consultant Obstetrician and your wife might also need admission in hospital. Ultrasonography is also required to confirm location of Placenta and the condition of baby. So, I strongly recommend you that you go to hospital as early as possible as second episode of bleeding are usually very heavy and dangerous.


Doctor, I’m 28 years of age and I already have 3 children. All were delivered normally and my youngest son is about 3 years of age. Now I’m again 7 months pregnant and my urine examination showed presence of sugar. Tell me medicines for this and is it dangerous condition??

Dear Patient,You have not mentioned in your previous question about the status of your blood sugar level, amount of urine sugar levels. Secondly you have not told me about blood sugar status in your previous deliveries, what were the weights of your previous babies, history of any diabetes in your family??? I strongly advise you that you should immediately consult your Obstetrician and you need to have complete blood sugar profile. After checking your blood sugar profile you may be put on inject able Insulin and dose of it will be calculated according to your blood sugar levels. Yes, Diabetes in Pregnancy is a highly dangerous condition and can lead to multiple abnormalities in baby besides putting the life of baby at stake. Diabetes in Pregnancy is also associated with infections in mother and can lead to abnormalities in Lab our. So, it is highly recommended that you should consult your concerned doctor.

Dear Patient, It’s entirely normal to experience some nausea and vomiting during pregnancy due to hormonal changes occurring during pregnancy. If you can’t tolerate this nausea or vomiting you try to take frequent small meals. Avoid large meals and eat small quantities of food at frequent intervals. You must undergo an ultrasound scan to rule out multiple pregnancy and molar pregnancy. Try to avoid any medicines.

To give you a complete advise I need your complete menstrual history including date of onset of menstrual periods, days of bleeding, after how many days does bleeding occur, amount of bleeding, is the menses associated with passage of clots, is menses associated with bearable or unbearable pain? Don’t be upset about this as most of the times this is only temporary phase and may be due to any stress or anxiety or anemia. After getting your complete history, I will advise you certain tests and then I may prescribe you certain medications depending upon results of your investigations.


I am a 26 year old woman. About 6 months ago, I had an ovarian cyst removed. It was benign. I recovered completely, no complications. Lately, I have been having similar problems as before my surgery. Tenderness in pelvic area and shortened menstruation. Can having a cyst make me more prone to recurrence or is this nothing to be concerned about? Thank you.

Dear Patient, Thanks for asking me! I would like to know in detail about your symptoms, previous obstetrical history if you are married, any history of infertility, Duration of menstrual bleeding, amount of bleeding, severity of pain and any other complaints! Do tell me some details of your previous operation whether it was only removal of cyst, condition of other ovary, was whole ovary removed in the previous operation?

I would suggest that you should undergo a detailed Ultrasound scan of your pelvis to look for the status of both ovaries and uterus and let me know the report. After all these queries and ultrasound report I would be able to respond to you! Thanks!


My sister is 6 weeks pregnant and suffering from nausea. She does not have hyper emesis gravidarium but does not eat enough due to the nausea. Eating makes it worse Can you advise any remedy so that she is at least able to maintain an adequate diet? Thank you.

Nausea and vomiting during pregnancy is mostly due to hyper emesis gravidarium. However, if you think it is not related to this condition, then other causes should be looked for. Meanwhile, try tablet. Navidoxin twice daily for vomiting before meals.


My wife gets painful periods. What could be wrong?

Dysmenorrhoea is a medical term used for painful periods. The condition is called 'primary Dysmenorrhoea' when no abnormality causing it is identified and 'secondary Dysmenorrhoea' when an underlying gynecological disorder is found to be the cause. An underlying gynecological disorder causing the symptoms (i.e. secondary Dysmenorrhoea) is diagnosed in only one-quarter of women with dysmenorrhoea.Several factors appear to worsen the pain of primary Dysmenorrhoea:

  • Having a backward-tilting uterus
  • Lack of exercise
  • Psychological stress
There can be different causes of Dysmenorrhoea. If it is not very severe it can be relieved by getting enough rest and sleep and exercising regularly. The pain can usually be alleviated most effectively with pain killers like brufen. If pain continues to interfere with normal activity, you should discuss this with Gynecologist.


After my wife underwent an exploratory laprotomy, the doctors ruled out TB Peritonitis. That was 2 months ago. Now, she underwent an ultra sound and found a lump in her ovary. Is it possible that this lump is still connected to Peritonitis as previously ruled out? I would appreciate medical suggestion as well as to what the best thing to do. Thanks!

Lump in ovary has no relationship with the peritonitis. Peritonitis (infection of abdominal lining peritoneum) is an acute emergency and requires urgent intervention. Lump can be a cyst or anything else.


what is Endometriosis? After surgery what are the chances of recurrence and is their a cure for this disease?

Dear Enquirer! The name endometriosis comes from the word "endometrium," which is the tissue that lines the inside of the uterus. If a woman is not pregnant, this tissue builds up and is shed each month when she has her period. In a woman with endometriosis, tissue that looks and acts like endometrial tissue is found outside the uterus, usually inside the abdominal cavity. Endometrial tissue that is found outside the uterus responds to the menstrual cycle in much the same way as the endometrium in the uterus responds. At the end of every cycle, when hormones cause the uterus to shed its endometrial lining, endometrial tissue growing outside the uterus will break apart and bleed. However, unlike menstrual fluid from the uterus, which is discharged from the body during menstruation, blood from the misplaced tissue has no place to go. Tissues surrounding the area of endometriosis may become inflamed or swollen. The inflammation may produce scar tissue around the area of endometriosis. These endometrial tissue sites may develop into what are called "lesions," "implants," "nodules," or "growths." Mild forms of endometriosis are common and may not require treatment. But, endometriosis can make it difficult for a woman to become pregnant. To answer the second part of your question, Yes there are chances of recurrence after surgery and there are different studies suggesting that recurrence can occur and there is no definitive treatment for that. But hormone pills like Progesterone (progestins,) Danazol (Danocrine)and GnRH agonists are helpful in controlling it.


Hi, Dr. I want to know from you that my wife is 2 months pregnant. Can I have sex with her? Or shall I have to wait for the delivery. If there in no problem having sex with her do I need some safety acts during that intercourse. As this one is our first chance to have a baby. Please let me know what to do. You can have sexual intercourse during pregnancy with a few concerns and no worries. The only "rule" is that sexual intercourse should be comfortable, pleasurable, and fun for both partners – and if it isn't, seek advice from your healthcare practitioner or a professional sexologist. Intercourse will not damage the fetus, nor will it induce premature labor.
